The program is for those 60 years of age and up or disabled who reside in Jefferson County.<\/p>\n<p>JCCOA is offering limited transportation services for routine medical appointments on the following days:<\/p>\n<ul>\n<li>Non-Emergency Medical Transportation(NEMT): Non-Emergency Medical Transportation at <span class=\"caps\">JCCOA<\/span> receives top priority!\n<ul>\n<li>Jefferson County: Monday-Friday\n<ul>\n<li>Appointments need to be made between the following times: Monday&#8217;s 10am-11am; Tuesday&#8217;s 10am-1pm; Wednesday&#8217;s 10am-12noon; Thursday&#8217;s 10am-1pm; Friday&#8217;s 10am-11am<\/li>\n<\/ul>\n<\/li>\n<li>Martinsburg: Wednesday&#8217;s ONLY\n<ul>\n<li>Appointments need to be made between the following times: 10am-2pm<\/li>\n<\/ul>\n<\/li>\n<li>Dialysis: Monday-Friday- All chairs times must be complete by 3pm.<\/li>\n<\/ul>\n<\/li>\n<li>\u00a0DHHR has recently decided to move to a statewide broker-managed Modivcare. Should you have Medicaid, please contact Modivcare at 1-844-549-8353 to schedule.<\/li>\n<li>Should you not have Medicaid and need to schedule an appointment, please contact\u00a0the JCCOA Main Line at 304-725-4044.<\/li>\n<\/ul>\n<p><a href=\"http:\/\/jccoa.org\/jccoa\/wp-content\/uploads\/2011\/07\/NEMT-sliding-scale-fee-1.pdf\">NEMT Sliding Scale Fee Schedule<\/a><\/p>\n<ul>\n<li>Non-Medical Transportation: <span class=\"caps\">JCCOA<\/span> provides non-medical transportation to shopping, recreation activities, hair appointments, banking, pharmacy purchases, etc. Monday-Friday. Non-medical transportation is provided on a donation basis.
