Lighthouse
The Lighthouse program provides support in four areas: 1) personal care-bathing, grooming, dressing, and toileting; 2) mobility-transferring in and out of bed or chair and assisting with walking when needed; 3) nutrition-meal preparation, feeding, if needed and grocery shopping; 4) environmental support- light housecleaning, laundry, making the bed and changing the linens.
There are 3 criteria:
A) Applicant must be 60+ years of age
B) Medically eligible based on a functional assessment by the provider’s Registered Nurse (RN)
C) Financially eligible, determined by a sliding scale fee schedule.
An individual can recieve up to (60) hours of Lighthouse services per month based on client assessment and resources available.
